On the fullness of my hands

Can someone tell me how the hell I’m supposed to respond to “You’ve got your hands full!”
I have two young kids. I suppose when I am holding each of their hands, then I suppose hands are full. I get this said to me practically every time we are out. Two kids isn’t even alot of kids. They aren’t twins, they are an average age apart.
But “you’ve sure got your hands full” isn’t a compliment. It’s barely an observation. It kinda feels a bit like an insult. What response are people hoping from this?
A guy yesterday YELLED it across the parking lot.
